Geriatric Care Management A Limited Liability Company (GCM) has been servicing the New Mexico community, and families from around the U.S., since 1994. In our 17 years of integrating services for seniors, our goal has been to maximize each person’s level of independence, while ensuring and enhancing their quality of life.
Our team provides personalized services designed to meet the needs of the geriatric population and our care managers have a minimum of 15 years’ of experience with geriatric services.
We begin by completing a comprehensive assessment or initial consultation for determining an individual’s specific needs. By doing so, we establish a baseline of information for aiding the clients and their families in making informed financial, legal, medical, and care decisions. This assessment also serves as a vital tool for new physicians and in case of emergencies. Our assessments are completed at the clients home, living facility, or hospital.
Our continuing care services have helped over 1000 seniors and their families in dealing with, tension, and frustration by providing needed information to help navigate through all the options of caring for a loved one. We understand how important it is to identify the appropriate level(s) of care. With this in mind, we help to coordinate any and all care needs and can be involved as needed or on an ongoing basis. Our services document important information needed for the provision of care and keeps track of any changes, particularly as pertains to medications, so that all information is up-to-date.
We serve as the elder's advocate in all needed interactions with physicians, facilities, families and/or advisors.
GCM facilitates decision making by acting as a referral source; making difficult decisions easier by skillfully identifying needs and solutions for our clients. Through our comprehensive assessment, needs are identified and specific resource information to meet those needs is provided and/or coordinated.
Other services provided include serving as Durable Health Care Power of Attorney or Court Appointed Guardian. We also have our "Just in Case" program for people planning ahead who do not currently need our services but want us to be ready to spring into action in a time of future need. One of our more popular services is our "Family Respite", which services local families who normally provide their parent's care. When they are away or vacationing, we fill in to be sure nothing falls through the cracks and coordinate while the adult children are on away.
